Insurance is provided by Harvard Pilgrim for group health and Delta Dental for dental. Annual Open Enrollment is in November every year.

Health Insurance

Employees located in one of the New England states – NH, MA, ME, VT, CT, and RI – have the option of selecting an HMO or a PPO. With the HMO plan, the insured will select a network primary care provider and obtain referrals to see a specialist. The PPO plan has a national network allowing the insured to use doctors anywhere in the USA. A primary care provider is not required and specialist visits do not require referrals.

Employees located outside of the New England area are not eligible to enroll in the HMO plan due to the network restrictions.

Eligibility for health insurance is the first of the month after the employee’s start date.

Below are the necessary forms and rate sheets, depending upon an employee’s place of residence.

Eligibility for dental and dependent vision insurance is the first of the month after the employee’s start date. Diagnostic and Preventative procedures are covered immediately and paid 100%; Restorative, Endodontics, Periodontics, and Oral Surgery are covered after a 6 month wait and paid 60%; Prosthodontics are covered after a 12 month wait and paid 50%. Eligibility for dental insurance is first of the month after the employee’s start date.
